Great work you’ll do here:
The Patient Care Coordinator provides clinical and management support to the Clinical Nursing Supervisor. The position assists the Clinical Nursing Supervisor to ensure quality and safe delivery of hospice care services and assumes responsibility for the day to day clinical operation of the Hospice program. This position may also assist with staffing and scheduling as needed. The Patient Care Coordinator may also help with admissions or nursing visits in times of high census or staffing shortages.
- Direct responsibility for delivery of quality care that meets client specific needs and in compliance with program/payer rules and physician orders
- Responsible for assisting clinical supervisor in efficient and effective utilization management in accordance with client needs, physician orders and payer requirements
- Facilitates communication and coordination of care between clients, families, caregivers, physicians and other service providers as necessary
- Assures efficient coordination of caregivers through case conferences and scheduling process as assigned
- Assists in monitoring patient records to ensure compliance with documentation requirements and program specific rules for coverage and qualifying criteria
- Assists in the QA process of medical records as delegated
- Assumes functions of Clinical Supervisor as required
- Provides orientation, training or in-services to new hires and the care team to assure the quality and appropriateness of care provided, on-site visits and competencies
- Assists in the completion of annual competencies and on-site visits for clinical staff
- As delegated, participates and assists in the data collection, analysis and reporting essential for the QAPI program
- Participates in and assists in the completion of monthly and quarterly audits
- Facilitates the audit process for clinical and billing purposes to improve the efficient delivery of client care and to assure appropriate reimbursement for services rendered
- Responsible for the correction of problems leading to denials for coverage or qualifying criteria
- Performs comprehensive nursing care to patient(s) including skilled nursing visits, coordination of care and establishing plans of care
- Assures thorough and accurate documentation with all clinical data including but not limited to: admission through discharge, physician’s orders, skilled nursing visits and changes in conditions
- Consistently demonstrates competency with technical nursing skills according to the legal scope of practice
- Implements patient specific teaching to patient/family needs and evaluates and modifies as needed
- Performs patient visits in an efficient manner, completing and submitting all documentation timely and per branch policy
- Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

- Graduate of an accredited school of nursing. Business coursework (business, finance, accounting or human resources) preferred in addition to a nursing license.
- Three (3) years of clinical experience required to consist of a minimum of: two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting.
- Current Registered Nurse licensure (in good standing) in the state(s) of practice
- Current CPR certification
- Valid driver’s license and automobile insurance
