This is a full-time, permanent position working as the Emergency Services Case Manager RN in the acute care hospital’s Emergency Department. The Emergency Services Nurse Case Manager will provide care coordination services to patients through cross collaborating with multidisciplinary teams, evaluating options and developing individualized patient discharge options. The Emergency Department Case Manager RN will join the Care Coordination team, working alongside a team of specialized case managers holding their CCM Commission for Case Manager Certification. Ideally the Emergency Department Case Manager RN will have previous Case Management experience working with an inpatient population within an acute care setting. Additionally, this individual will be up to date on best practices, state, federal and hospital regulatory compliance and guidelines, have prior discharge planning assessment and utilization management experience, and hold an active registered nurse license.
Communication is key for the Emergency Department Case Manager RN when presenting information to the patients and families. The Emergency Department Case Manager must hold their NYS Registered Nurse license prior to starting, PRI Certification, have inpatient hospital experience ideally working in an Emergency Department, along with previous managed care experience performing utilization review and discharge planning.
