The Quality Improvement (QI) Registered Nurse develops, assures implementation, and analyzes all aspects of quality assurance and improvement procedures. The QI Registered Nurse is responsible for ensuring that patients are receiving the highest level of care that will promote positive patient outcomes while following CMS regulations.
- Maintain compliance with Federal and State regulations and follows applicable accreditation standards in all Quality Assurance and Improvement activities.
- Maintain compliance in accordance with company policies and procedures.
- Ensure the establishment of patient centered care plans from admission to discharge.
- Ensure timely request of authorizations.
- Evaluate the quality and effectiveness of nursing practices and nursing services, analyze appropriate data to identify opportunities for collaboration with all stakeholders to improve services and patient outcome.
- Serve on the company committees as designated by agency leadership.
- Assist in the orientation, training, and education of clinical field staff.
- Maintain and assess all aspects of QAPI.
- Monitors EMR Reports relating to Oasis, POC and RAP timeliness and assists managers with strategies to meet deadlines.
- Serve as a resource to staff regarding performance improvement in all areas to include, but not limited to accuracy of information, supporting documentation including focus on OASIS assessments and timeliness, plan of care development and overall job performance.
- Export OASIS assessments for specified branch locations weekly and as needed and maintain Validation Reports of OASIS transmissions in the Teams folder. Identifies patterns of error, staff performance errors, recommends action measures and educational needs.
- Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered sensitive to socio-cultural diversity.
- Participate in on-call for after-hour client care.
- Promote an environment of quality and safe client care through participation, development, and adherence to the QA plan and associated activities and metrics.
- Perform other duties and accept responsibilities as deemed necessary.
Qualifications:
- Current license as an RN.
- Minimum of five (5) years’ experience in home health.
- Thorough knowledge of Federal and State regulations.
- Good organizational and communication skills.
- Minimum 0f 1-2 years of OASIS and home health documentation review.
- Ability to travel approximately 25% of time
