The Ambulatory Pediatrics Nurse Navigator is responsible for analyzing and supporting quality needs, evaluating opportunities for implementing initiatives to improve patient outcomes and meet regulatory requirements.As a member of the interdisciplinary team, the nurse navigator contributes professional nursing knowledge and skills in the provision and management of care to pediatric patients through the application of the nursing process and quality improvement.Works collaboratively with other pediatrics departments and those who are involved with pediatric care to ensure an organized, cost-effective, and quality-driven approach to the care of pediatric patients.This position will assist with identifying gaps in processes and facilitates process improvement activities to improve outcomes.Implements evidence-based research into practice to ensure patients benefit from the latest innovations in the delivery of care.The Ambulatory Pediatrics Nurse Navigator serves to increase the driving force and consistency of quality and safety at a department and service line level.
- Ability to balance multiple tasks/projects while meeting organizational standards
- Utilizing data metrics to prioritize and drive practice change by uitilizing performance improvement methodologies (PDSA) to implement change
- Identifies essential relationships and variations in clinical practice for the pediatric patient population. Evaluates current practices and implements evidence-based practice recommendations for change.
- Collaborates with the unit leaders, senior leaders, providers, department staff and others to address issues related to patient care and patient care outcomes and to ensure consistency across the continum of care
- Facilities the development, provision, and conduction of clinical education, quality improvement, and policy development within the service line
- Serves as a role model and resource for the nursing division shared governance model. Serves as a liasion for the service line for new program development and improvement initiatives.
- Review overall compliance by staff on regulatory agencies related to patient safety
- Writes and reviews department-based policies, procedures, and standards to reflect the latest evidence
- Tracks and reports quality outcomes and unit trends to management, physician teams and department nurses
- Provides real time accountability, coaching, education to promote ownership of safe practice
- Completes and equivalent of at least 8 hours of direct patient care hours per week
- Promotes transparency of data and accountability by providing quality reports to unit leaders and department staff on a daily, weekly, monthly, quarterly, and yearly basis
- Participate in projects and tasks to promotes efficient use of resources, identify and recommend action to improve operations and processes
- Perform other duties as assigned including tasks if needed during times of high volumes in the clinics
- Displays effective facilitation skills and maintains sold relationships with leaders and key stakeholders to ensure effective completion of project deliverables
