Position Purpose: Crisis Co Responder Team is recovery and resiliency oriented, intensive, community based rehabilitation and outreach. It is team based and consists of mental health rehabilitation interventions and supports necessary to assist the recipient in achieving and maintaining rehabilitative, resiliency and recovery goals. Interventions are provided in natural settings, and are delivered face to face, by telephone, or by video conference with individual recipients and their family/significant others to the primary well- being and benefit of the recipient. Provide program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards.
Qualifications:
- Bachelor's degree in social work, psychology, or human services required. Master's preferred.
- Minimum: Mental Health Profession (MHP) or (QMHP).
- Trained in crisis intervention, experience in the mental health field is required.
- Excellent clinical and diagnostic skills
- Able to resolve conflicts with internal and external customers.
- Competent in handling psychiatric crisis
- Excellent written and verbal communication skills
- Excellent organizational skills, able to manage time and meet deadlines
- Competent computer skills
- Self-starter with ability to work independently
- Valid Driver’s License
Essential Responsibilities:
- Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.
- Phone and/or face-to-face contact with adults in a psychiatric crisis, and knowledgeable of effective crisis intervention techniques to assist in stabilization of psychiatric crisis.
- Attend Court calls to reach hard to find clients.
- Seeing clients while they are in jail to attempt to connect them with services.
- Work alongside law enforcement officers.
- Respond to live or active 911 calls with Law Enforcement that involve clients experiencing mental health crises.
- Assist Law Enforcement Officers with de-escalation during crisis calls.
- Create a disposition including referral and linkage, which may include crisis planning, crisis residential, triage center, return home.
- Provide comprehensive diagnostic assessment to independently determine provisional diagnosis and the medical necessity for services.
- Determine appropriate level of care and make a referral accordingly.
- Obtain all client information needed for referral and necessary paperwork for Rosecrance Ware Center.
- Arrange for transportation as necessary.
- Provide Crisis Intervention until transport arrives if medically necessary.
- Arrange linkage with hospital or Crisis Residential.
- Refer consumers to agencies which specialize in problems other than mental illness, i.e. alcohol/drug abuse, domestic violence, etc.
Schedule:
- Tuesday - Friday from 1:00pm-11:00pm
- Rotating weekends
Work Location: Rosecrance Ware-Rockford, IL
Pay rate: based on education and experience
- 2nd Shift differential: $1.00/hr
