Position Purpose:
This Outreach Worker position has the focus of entering data into the Homeless Management Information System (HMIS) data system as provided by the Projects for Assistance in Transition from Homelessness (PATH) team. The Outreach Worker will be responsible for keeping up to date data in the HMIS system and will be responsible for reconciling data completeness reports. The Outreach Worker will also participate in outreach activities in the community and assist with the coordination of care for individuals in the homeless and housing programs. This position works directly with the data management system as well as directly with individuals in the program.
