Introduction
Medical City Dallas Transplant Institute is a growing and dynamic transplant program. We offer Liver, Kidney and Pancreas Transplant here at Medical City Dallas and Heart Transplant at Medical City Dallas Heart and Spine Hospital. Currently, it is the only program in the region with both adult and pediatric programs in one hospital. Medical City Dallas has a long history of clinical excellence in Kidney and Pancreas Transplant. In 2022, we opened our new Liver Transplant Program and have had great success and growth. We anticipate continued growth and continue excellent outcomes in our Liver Transplant program.
We are looking for a Transplant Pharmacist to join our dedicated multi-disciplinary teams. You will work closely with our Transplant Surgeons and our staff to provide excellent clinical care and support for our patients. Job Summary and Qualifications
The Advanced Clinical Pharmacist – Solid Organ Transplant is responsible for providing all levels of pharmacy clinical services for the solid organ transplant patient. This includes monitoring patient medication regimens and providing appropriate interventions as well as supporting all aspects of pharmacy operations.
What You Will Do in This Role
- Provide pharmaceutical care by active participation in multidisciplinary rounds; designing, implementing and monitoring patient-specific pharmacotherapy, including pharmacokinetic support, nutrition support and documenting pharmaceutical care activities
- Participate in selection committee(s) related to the care of kidney and pancreas transplant patients.
- Assist in the development, implementation and monitoring of clinical pharmacy services in Solid Organ Transplant in both the inpatient and outpatient setting;
- Ensure and promote the safe, rational, efficacious and cost-effective use of drug therapy
- Provide leadership for Solid Organ Transplant services including developing guidelines and policies, staff training activities, developing quality improvement processes, and to assist in the clinical training of patients and staff
- This position is designed with a focus on abdominal organ transplant with split inpatient and outpatient responsibilities for kidney and pancreas transplant patients.
- Assist in the development of staff educational and training programs within and relating to the solid organ transplant service line.
- Provide drug information and education to patients and caregivers.
- Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies
- Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations
- Provide patient/family counseling and education
- Reconcile home medications with medication orders in the hospital and at discharge
- Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care between the inpatient and ambulatory settings
- Monitor and modify patient medication therapy in collaboration with the medical team achieve desired therapeutic outcomes in the ambulatory setting
- Interpret orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system.
- Complies with facility policy and procedures and regulatory requirements.
- Support and participate in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
- Complete notification reports regarding medication errors and adverse drug reactions.
- Has knowledge of the patient populations served and apply that knowledge in the care of those patients.
- Provide basic technical support for automated dispensing machines and other pharmacy computer systems.
- Compliant with facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses and other members of the healthcare team.
What Qualifications You Will Need:
- B.S or PharmD from an ACPE-accredited School of Pharmacy, A Doctor of Pharmacy from an ACPE-accredited College of Pharmacy is preferred
- Active Pharmacist license in good standing with the TX State Board of Pharmacy
- At least one of the following: Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required, preferably within area of focus
- Completion of hospital PGY1/pharmacy general practice residency or fellowship
- Specialty BCPS certification
- Other certification (e.g. FCCP or FASHP)
