RN CASE MANAGER - CARROLL HOSPITAL - PART TIME

JOB SUMMARY

The primary emphasis of the RN Case Manager is to partner with all other members of the health care team to assure efficient care of inpatients, avoiding delays and denials whenever possible. The Case manager, working with health care team, strives to achieve the best outcomes of appropriate utilization and safe, efficient and timely transition and discharge planning functions.

SCHEDULE: 3, 8-hour shifts per week (24 hrs.), Mondays, Tuesdays, and Wednesdays.

REQUIREMENTS

• Associate's Degree in Nursing required, but Bachelor's in Nursing preferred.

• Min. of 2-4 years related experience and/or training or equivalent combination of educational experience required

• Utilization Management, Case Management, and/or Discharge Planning experience preferred

• CCM - Certified Case Manager required within 3 years of hire

• CPR - AHA Healthcare Provider required

• RN - Registered Nurse required

• ACM - American Case Management Certification preferred
