• Dives LifeBridge’s Mobile Clinic van--including on some Saturdays and Sundays--to help ensure needed services reach underserved residents where they live.
• Provides direct service to residents of neighborhoods and communities in the LifeBridge Health service areas by means of health screening, referrals to clinical and health-supporting social resources, education, health insurance sign-up, and longitudinal coordination for residents to obtain the services they need to achieve and maintain good health
• This position directly supports LifeBridge’s overall aim of improving health and wellness in our communities and minimizing avoidable hospitalizations and Emergency Department use

JOB REQUIREMENTS:

• Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field
• 1-3 years of experience
• Must have full MD driver's license and good driving record
• Will be required to work some evenings and weekends
