Position Overview
We’re seeking a talented and caring per diem Physical Therapist (PT) for our outpatient team in Torrington.This is a great opportunity to work with a diverse patient population with a wonderful team atmosphere with experienced clinicians. Specialties in this office include LSVT, vestibular and lymphedema treatment.
The Physical Therapist (PT) administers medically prescribed Physical Therapy treatments to patients. Independently evaluates patient’s level of function, determines treatment strategies, and delivers and/or delegates Physical Therapy services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain.
This role encompasses evaluation skills and techniques, analysis of patients, and therapeutic application of physical and chemical agents, prescription of exercise and other procedures to maximize the patient's functional independence.
Essential Functions
- Performs a comprehensive physical, psychosocial, and safety assessment of assigned patients as evidenced by clinical documentation.
- Develops a Plan of Care to promote optimum function and safety as documented in the clinical record.
- Plans for and provides skilled physical therapy treatment, reassessing the patient’s status on each visit and ensuring quality of care as documented in the clinical record.
- Coordinates patient care with other clinical team members ensuring optimal patient care and communication as noted in the clinical record.
- Completes all required documentation following Hospital policies and procedures to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record.
- Maintains and enhances personal knowledge as well as promoting professional standards in physical therapy.
- Attends all mandatory in-services, completes all mandatory Health stream assignments.
- Follows the Hospital isolation procedures.
- Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
- Promotes effective working relations and works effectively as part of a team inter- and intra-departmentally to facilitate the department's ability to meet its goals and objectives.
