POSITION SUMMARY/PURPOSE
The Advanced Practice Registered Nurse is a certified clinical nurse specialist in adult, family practice or pediatric practice who is responsible for providing psychiatric treatment including: psychiatric evaluations; diagnosis; prescribing; psychotherapy; coordination of care, and patient education in all setting of the Department of psychiatry including Inpatient, Emergency Department Crisis Intervention, Partial Hospital, Outpatient and in community settings where the department is contracted to provide services. The APRN works closely with the Outpatient Medical Director, staff psychiatrists, and clinicians in a patient centered multidisciplinary treatment team approach. The APRN has prescriptive authority and licensure as an Advanced Practice Registered Nurse through the State of CT Department of Public Health and is credentialed through the medical staff of CHH.
ESSENTIAL FUNCTIONS
- Evaluate patients through detailed psychiatric evaluations that include mental status and risk assessments, a full review of psychoasocial comprehensive assessments, and development of a detailed medical history.
- Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with primary care physicians and medical specialists.
- Develops and implements a treatment plan that involves and includes the patient in active participation in treatment. And revises the plan as indicated. Provides monitoring and continuity of care between visits according to treatment plan including triaging patients calls and communications.
- Writes orders for medications, treatments and other matters pertaining to patients care.
- Writes progress notes on patient’s charts indicating patient status and treatment recommended.
- Provides psychotherapy to individuals or groups as appropriate and requested by the Medical Director.
- Assumes care for physical and psychological needs of patients during examinations and provides emotional support to family members when appropriate.
- Charts observations, test results and all patient communication in the medical record as directed by CHH hospital policy and procedure.
- Ensures accurate maintenance of patient’s medical records and that patient records are treated as confidential information.
- Advises patients and families on use of various medication including dosage, side effects and composition.
- Reviews and interprets results of diagnostic studies and tests, consults with supervisory physician when appropriate, incorporates information into plan of care.
- Determines and discusses diagnosis with patient, documents information on charts and charge slips.
- Arranges for appropriate follow-up care/referrals for patients.
- Oversees R.N. services and operations of the department as assigned by the Medical Director.
- Provides healthcare diagnosis discharge information for the patients and families and writes discharge instructions.
- Recommends and develops new services and programs.
- Participates in peer review, chart reviews, staff education, clinical guidelines development, and other continuing education and quality assurance activities to demonstrate compliance with standards.

JOB KNOWLEDGE, SKILL REQUIREMENTS, AND DESIRED ATTRIBUTES

Knowledge of advanced practice of nursing including anatomy and physiology, psychiatry, psychopharmacology, psychosocial/family systems, drug and alcohol addiction, diagnostic testing, interpretation of results and clinical decision making.

Knowledge of professional nursing theory, practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery of nursing care.

Maintains good relations and fosters effective public relations with patients, medical staff and other departments as well as other hospitals that use services.

Maintains established departmental policies and procedures, objectives, quality improvement program, safety, environmental and infection control standards and participates in assigned quality improvement efforts and development of new procedures for patient care.

Enhances professional growth and development through participation in educational programs, current literature, in service meetings, and workshops and incorporates information for improving patient care.

Attends clinical disposition, supervision, and case review meetings as required.

Meets the established quality and productivity measure of the department.

Performs all other related duties as assigned.
Qualifications
EDUCATION
The job requires a Master’s or other advanced degree in Nursing/APRN. NP certification and CPR certification required. The job also requires a CT APRN. DEA/CT Controlled Substance required.
