Job Description
Provides quality, cost-effective inpatient/outpatient and/or community care for clients and families in a target population across the continuum of care. Works in coordination with interdisciplinary teams to organize, prepare and deliver evidence based clinical services reflected by a multi-disciplinary plan of care.
Job Responsibilities
- Evaluates the psychiatric and psychosocial needs of the patient/client to assess and plan care. Establishes a multidisciplinary plan to address identified issues and goals.
- Utilizes individual, couple, group, and family therapy specific to address each patient’s/client’s needs as part of the multidisciplinary treatment team in accordance with the treatment plan. Organizes and prioritizes care, interventions, safety planning, and treatment. Identifies and stewards appropriate resources in order to meet patient/client and family goals.
- Provides client medical documentation based on program needs including but not limited to social history, treatment plans, progress notes, discharge, assessment, and After Care Plan in accordance with regulatory licensing requirements and third party agencies. May diagnose patients/clients using indicated DSM criteria.
- Coordinates and implements client/patient/family care along the continuum by collaborating with the multidisciplinary team, payers, community and regulatory agencies to provide an integrated delivery of healthcare.
- Educates patient/client and family members and ensures seamless, coordinated transitions across levels of care to meet established goals. Refers patient/client and families to appropriate levels of care, including support groups and to other available resources in order to facilitate recovery and/or treatment process.
- Provides direct patient/client care including individual, couples, family therapy, group therapy, advocacy, referral and discharge planning. Performs case management and case coordination: arranges aftercare and other necessary services.
- Regularly evaluates goal directed effectiveness and efficiency of plan, documents patient/family responses, identifies variances from outcomes and revises plan of care to meet patient/family needs.
- Negotiates level of care and length of stay with managed care companies and informs the third party payers of ongoing changes in the client’s status.
- Works across programs for clients with both mental health and substance use disorders. Demonstrates competencies, including knowledge and behaviors, to meet age specific patient needs. Ensures personal, patient/client and unit/staff safety. Adheres to entity policies and procedures, reports and responds to risk management issues in a timely manner to safeguard the entity from adverse events.
- Participates in administrative and development activities including team meetings, clinical supervision of staff and staff education. Supports the development of hospital and/or unit based programming to meet the clinical and strategic needs of the clients/patients and the milieu. Works effectively as a team member to contribute to improved patient/client experience. Ensures the cost-effective utilization of resources.
- Perform other related duties as required which may include but are not limited to: Providing supervision and/or clinical supervision for interns and other staff as assigned. Providing on-call coverage.
Qualifications
- Master’s Degree in Social Work, Counseling, or Related Field
- Three years’ experience providing direct services to psychiatric patients, substance use population.
- Previous experience in assessment and diagnosis and individual/group therapy and crisis intervention.
- Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, Marital & Family Therapy.
