The Care Coordinator Nurse Navigator demonstrates expertise in a healthcare setting in the assessment and treatment of patients along the continuum of care. The navigator identifies patients who meet criteria for their disease specialty through risk-screening and referral process. Education is provided to patients and families during the acute care phase and post-acute follow up for 30 days with each patient in order to reduce the risk of readmission and ensure that post-acute care needs are being addressed. Provide education to the healthcare team members on their role and its impact in the care of the patients. Collaboration and relationship building with all agencies and providers serving the disease population along the continuum of care. The navigator will serve as the primary point of contact for the patient care coordination along the continuum.

Education
Bachelor in Health-Related Field
Experience
2 years experience in acute care healthcare setting.
Licensure, Certification, Registration
Valid licensure or registration in Healthcare Related Field of Expertise (ie RT, medical administration)
Preferred bilingual English/Spanish
