Job Summary
Under the supervision of the Clinical Manager of Out-Patient Behavioral Health, this role will be expected to utilize a high degree of clinical acumen and is responsible for the triaging of referrals and assignment of cases across multiple programs. This role requires a wide knowledge base to quickly determine the clinically appropriate services to meet patient need. This candidate will have a thorough knowledge of the available continuum of care, as well as, a nuanced understanding of the psychiatric diagnoses associated with child and adolescent mental health. This pivotal role will be expected to manage a small personal caseload and be versatile in supporting the daily clinical operations. The triage clinician will have opportunities tocollaboratively influence clinical program development based on the assessment of clients’ and families’ needs.
Job Responsibilities

- Receives all telephone referrals from patients and providers. Provides clinical mental health telephone triage for calls and assignment to appropriate program and level of care.
- Conducts scheduled and walk-in comprehensive admission assessments with patients that include the collection of psychiatric, family, medical, substance abuse, development, social, work, and school history through face to face clinical interviews of individuals, families, or guardians. The comprehensive assessment incorporates results of clinical assessment tools and prior psychiatric/medical records.
- Preliminary clinical multi-axial diagnostic impressions utilizing the Diagnostic Statistical Manual for Psychiatric Disorders/ICD at the time of each comprehensive assessment and treatment plan.
- Dictated/written documentation of comprehensive assessments, treatment plans, progress notes, and case management collateral contacts that meet established Joint Commission, state regulatory and corporate compliance criteria.
- Insure through benefit eligibility verification that behavioral health managed care insurance for each patient is individually managed to minimize loss of revenue.
- Utilizing the Meditech System, schedules patients into Meditech in agreed upon time slots.
- Collects and organizes data as assigned related to Charlotte Hungerford Process Improvement Measures, patient surveys and projects assigned by the Clinical Manager or Administrative Director.
- Appropriately keeps all records organized and maintained as outlined by Clinical Manager and Administrative Director.
- Provides all assigned clinical and administrative support for clinicians to meet the case management needs of patients while assisting clinicians with tasks that enhance clinical productivity.
- Interacts with patients and their family members present for outpatient mental health treatment in a manner that engages family in services and embodies principles of recovery.
- Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job-related hazards. Follows the home and community services division exposure control plans/blood borne and airborne pathogens.
- Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmental to facilitate the department's/unit's ability to meet its goals and objectives.

Qualifications
Qualifications
- Master's degree in Social Work, Marriage and Family Therapy, or Counseling
- Active LCSW, LPC, or LMFT license required
- Two years clinical experience in a psychiatric setting
