Hartford Healthcare is seeking a Nurse Practitioner or Physician Assistant for highly regarded, collegial, and collaborative Transplant Clinic practice. The APP would see patients in outpatient setting.
Role: Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistant/Physician Associates (PA-C)
Location: Outpatient Offices are located in Hartford
Setting: Outpatient responsibilities in clinic
Schedule: 40 hours per week; Occasional weekends and holidays are required
Nurse Practitioner and Physician Assistant Role Scope:
·The Outpatient Post-Transplant Midlevel Practitioner will provide comprehensive medical care for patients who have undergone kidney or other solid organ transplants.
·They will work in an outpatient setting and focus on managing post-transplant care, including monitoring patient progress, managing immunosuppressive therapy, and coordinating follow-up care.
·Conduct outpatient clinic visits for post-transplant patients, including taking medical histories, performing physical examinations, and ordering diagnostic tests.
·Address acute and chronic post-transplant issues, including infection prevention, rejection monitoring, and medication side effects.
·Collaborate with transplant surgeons, nephrologists, nurses, social workers, and other healthcare professionals to ensure comprehensive patient care.
·Coordinate follow-up care, including scheduling appointments, managing referrals, and communicating with other specialists.
·Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and communication with other healthcare providers.
·Ensure all documentation meets regulatory requirements and institutional policies. Participate in quality improvement initiatives and contribute to developing best practices for post-transplant care. Stay current with advancements in transplant medicine through continuing education and professional development opportunities.
Qualifications
·Licensure: Unrestricted State of Connecticut Medical License (or eligible)
·Training: APRN (Family, Pediatric, Adult/Geri Nurse Practitioner) or PA
·Experience: At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) a plus – not a must.
