All 26 beds are in private rooms giving our patients access to ongoing acute medical management, 24 hour rehabilitative nursing care and the latest rehab therapies. Our dedication to delivering personalized coordinated care coupled with the healing environment offered by this new unit will shorten length of stay and allow our patients a quicker and healthier return to home. Overview We’re seeking dedicated and caring Case Coordinators to be part of a team with a patient-centered approach, in providing the most innovative care. This specialized role will assess transition needs of patients with a focus on stroke, traumatic brain, spinal cord injuries, along with other eligible diagnoses. The Case Coordinator will facilitate appropriate services across the care continuum, with the goal of returning the patient to their home environment. Our new unit features a collaborative extraprofessional approach to patient care. This is an excellent opportunity to work in a specialized inpatient rehab setting, while still having access to various providers within an acute care hospital.
Qualifications
• Current state licensure or eligibility for state licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT) or Speech Therapist (ST) required. • One year of diversified Case Management, home care, and/or managed care experience. • Inpatient rehabilitation experience preferred. • Certification in Case Management strongly preferred.We take great care of careers.With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
