Summary
The Complex Case Manager provides complex case coordination services as a member of the Case Coordination and Social Work Department and interdisciplinary professional team. This position must demonstrate strong advocacy skills with a focus on problem solving and creating collaborative plans for positive patient outcomes and transitions.
This role focuses on complex patients both social and medical with an anticipated long length of stay (>10 days) and complex transition planning needs.
Responsibilities
·Performs transition of care assessments and care planning for complex medical and social patients in the Fairfield Region, which involves patients and families in transition planning.
·Identifies transition planning, problem solving and barriers in documentation
·Collaborates with the primary CM, social worker, preventative medicine, palliative, medical and nursing teams to problem solve barriers to discharge that may include complex medical and social needs. Bridges care with ICP teams or other agency teams to prevent readmission.
·Connects with key stakeholders in the community to understand resources and services that could help transition patients to the community or supportive living situation. Identifies resources within HHC system and larger community to address patient care needs.
·Provides education for patients, families, community, and other professionals regarding disease prevention, impact of illness and advocacy for benefits and health maintenance.
·Keeps patient panel statistics that highlight decreased LOS, collaborative work relationships and decreased readmissions and program evaluation.
·Perform other related duties as required.
Qualifications
·BSN required.MSN preferred.
·Minimum of 5 years’ experience in hospital based case coordination.10 years preferred.
·At least five years of experience working in an acute care setting.
·Excellent communication skills (written & verbal).
·A passion for helping vulnerable populations.
·Highly effective analytical, organizational and problem solving skills.
·Positive, customer-focused approach.
·Ability to work independently and with teams.

·Strong facilitation and statistical skills.
·Knowledge of federal & state regulatory requirements.
·Strong knowledge of Lean and performance improvement methodologies

·Strong ability to provide expert verbal and written clinical documentation and consultation along the continuum of care.
·Must be able to work collaboratively, efficiently and effectively with multidisciplinary health care professionals to ensure a seamless transition of care for our patients and families.
·Ability to multi-task and address multiple needs of healthcare team members and patients/families.
·Excellent ability to address complex psychosocial needs by working with community resources and addressing barriers that prevent patient from optimizing their health and quality of life.
·Ability to work in fast changing healthcare environment.
·Must maintain CEU’s as required for RN, CCM and/or ACM licensure.
·Physical abilities to include sitting and standing for prolonged periods.
·Adheres to HIPPA requirements.
