Compensation will be determined based on years of experience and applicable certifications.The Senior Case Manager assists the administrative manager in providing training, coordination and direction in the day to day function of the department of Case Management. The Senior Case Manager collaborates with Senior Leadership regarding patient flow, census management, patient care outcomes and financial performance indicators, and provides direct case management services. The Senior Case Manager oversees the effective coordination of services in the areas of insurance relations, fiscal management, admission and discharge coordination, case conferencing and communication relative to the overall treatment plan. As the interdisciplinary team coordinator, the Senior Case Manager facilitates communication amongst team members regarding delivery of services to patients and families, performs ongoing utilization review, acts as a liaison to the payor, assures cost effective treatment, facilitates timely communication and implements the discharge plan. The Senior Case Manager can work in either the inpatient or outpatient setting.Job Code: 100157
QualificationsLicense or Certification:- Maintains current required state licensure based on education, and national certification if required by professional background. Certifications (CRRN, CIRS, CRC, ACM or CCM) preferredEducation, Training and Years of Experience:- Bachelors or masters degree preferred in social work, rehabilitation or related allied health field, or a degree in nursing (BSN preferred) with appropriate state licensure/certificationsMachines, Equipment Used:- General office equipment such as telephone, copy machine, fax machine, calculator, computer.Physical Requirements: - Good visual acuity and ability to communicate.- Ability to lift, lower, push, pull, and retrieve objects weighing a minimum of 30 pounds.
