NEW HIRE SIGN ON BONUS ELIGIBLE POSITION - $5K! # General Summary of Duties: Responsible for the accurate process of referral/intake services. # Supervision Received: Reports directly to the Referral Center Coordinator and Vice President of Business Development. # Supervision Exercised: None. # Typical Physical Demands:# Requires prolonged sitting, requires some standing, bending or stooping.# Requires eye-hand coordination and manual dexterity.# Requires the ability to lift a minimum of 30 pounds.# Requires the ability to ascend/descend stairs.# Requires the ability to distinguish letters, numbers and symbols.# Requires corrected vision and hearing to normal range. # Typical Work Conditions: Work is performed in an office environment.# Contact with Agency staff and community. # Essential Duties: (This list may not include all of the duties assigned.) # 1.################ Responsible for identifying and obtaining all necessary information for referrals. a) Assess W10 and telephone calls from referral sources for appropriateness for home ############ ##### care. b) Accurately completes the OASIS intake form. c) Obtains a verbal order for SOC that includes a description of the patients condition ######################### and the services to be provided by the Agency and meets HCFA regulations. d) Assigns accurate ICD codes to diagnosis. e)# Coordinate all disciplines/services for successful initial assessment. f)# Scans referral data to appropriate departments and supervisors. # 2.################ Responsible for obtaining accurate insurance information. a) Assesses eligibility for agency programs b) Obtains prior authorization when necessary c) Maintains log of hospitalized patients############# ## d) Demonstrates knowledge of computer programs. ie.
