About the unit
Our Visiting Nurses Association is our Home Care Skilled Nursing Team for West-Central areas of Michigan. We are accepting applications from experienced RN’s, and LPN's. This Remote Patient Monitoring RN will support both our regular Remote Patient Monitoring and Transition to Home nurses providing 24/7 monitoring to patients to help prevent avoidable emergency room visits and hospital readmissions. This is considered a flexible remote position where training will be onsite in Grand Rapids and work location may vary depending on program needs but once independent in the role, the nurse can predominantly work remotely . In addition to training needs and orientation schedule, there is an expectation of some required in person meetings in Grand Rapids. A share of the weekend and holiday rotation are required.
Scope of work
Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
