Work of scope
The Serious Illness Care Navigator (SICN) will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. Demonstrates expertise, current knowledge in care management of a caseload of patients with high complexity and seeks to improve patient, family and health systems /community outcomes through the application of education concepts/skills and planning for medical crisis and endof life in a management care environment. SICNs will have visits with patients and families while hospitalized, in post-acute (skilled nursing facilities) and completed home visits to manage patient goals and needs for their health care.
