Job Summary
The Medical Assistant Coach is a member of the interdisciplinary healthcare team seeking to optimize patient health outcomes in a population-based approach. Under the supervision of a licensed healthcare professional (physician, advanced practice provider, or registered nurse), the Medical Assistant Coach conducts outreach to patients with identified gaps in care and supports patient/client healthy behaviors. The Medical Assistant Coach provides direct care support for a patient's clinic journey from check-in of patient visits to check-out of patient visit. This role is active in quality improvement through daily support with problem solving and resource allocation, clarifying root causes and driving cross functional problem solving in the team.
Essential Functions
- Works under the supervision and delegation of a licensed healthcare provider (physician, advanced practice provider, or registered nurse) to serve an assigned panel of patients.
- Builds a trusting patient partnership to motivate self-efficacy, readiness for healthy change, and engagement to the care plan.
- Engages in population-based panel activities through the review of health registries at regular intervals. Registries are reviewed to ensure all tasks related to preventative and chronic care are performed.
- Provides in person and telephonic outreach to close gaps in care. Follows standard work to call patients/ send letters/ send Myhealth messages with preventative and chronic gaps.
- Supports care coordination by calling patients post emergency room visit to ensure they have a follow up appointment scheduled.
- Responsible for calling newly assigned/ attributed patients from payer lists for initial appointment. Call patients who have not been see in compliance with tiering protocol to schedule follow up appointment
- Perform empanelment clean up: compare payer files and internal reporting to ensure the proper empaneled PCP is listed in general PCP field and act, as necessary.
- Provides schedule management for designated medical providers ensuring patients see the right provider at the right time. Ensures patient flow is maintained. Proficient in MDI, and able to ensure employee conformance to standard work and daily lead metrics. Responsible for daily improvement and care team tracking center.
- Exhibits understanding and promotes the practice of safety and infection control standards, and ensures employee conformance.
Qualifications
- Required High School Diploma or equivalent
- Preferred Associate's Degree
- Successful completion of a competency based orientation program Required
- 2 years of relevant experience working in a physician practice or healthcare setting Required
- Experience working as a medical assistant Required
- Experience in closing patient care gaps and improving quality health outcomes Preferred
- Experience in motivational interviewing and influencing healthy behavioral changes Preferred
- Experience in Lean Quality Improvement Preferred
- CRT-Medical Assistant, Certified (CMA) - AAMA American Association of Medical Assistants Upon Hire required Or
- CRT-Medical Assistant, Certified (CCMA) - NHA National Healthcareer Association Upon Hire required Or
- CRT-Medical Assistant, Certified (NCMA) - NCCT National Center for Competency Testing Upon Hire required Or
- CRT-Medical Assistant, Certified (NRCMA) - NAHP National Association for Health Professionals Upon Hire required Or
- CRT-Medical Assistant, Registered (RMA) - AMT American Medical Technologists Upon Hire required Or
- CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or
- CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required
- CRT-Health and Wellness Coach, Certified (CHWC) - ICF International Coach Federation Upon Hire preferred Or
- CRT-Health and Wellness Coach, Certified (CHWC) - NBHWC National Board for Health & Wellness Coaching Upon Hire preferred
