Licensed Vocational Nurse (LVN) with the largest private employer in the Central Valley, the average day is anything but average and you will actively feel and see how your role impacts our mission and those we serve.CSTCC is a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision. CSTCC has two very unique and distinct units.Our 58-bed subacute unit is medically certified as a rehabilitation respiratory unit. We specialize in trach weaning, helping patients come off of vents, feeding tubes, retraining patients to eat and breathe. The average length of stay for patients in subacute is about 6 months to one year.Our 48-bed short-term skilled unit provides wound care, IV therapy, and focuses on getting the patients transitioned back to their lives and community. This unit houses a full rehabilitation gym for our patients.
Your LVN Career at Community | Opportunity. Challenge. Growth. Responsibilities: Accountabilities1. Assesses the resident's condition and nursing needs. Prescribes appropriate nursing action on admission and as needed.2. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.3. Meets current documentation standards and policies including use of all facility documentation forms properly.4. Notifies physician of changes in resident's condition and follows through until appropriate action is taken including proper documentation, nurses' notes and care plan.5. Notes and carries out physician's orders in a timely manner.6. Follows through as needed on information given by shift report, resident or family regarding resident concerns.7. Demonstrates competence in technical and manual skills according to scope of practice for LVNs. Demonstrates eagerness in learning new skills.8. Administers medications and follows the five medication rights and reduces the potential for medication errors.9. Interacts with residents, families, co-workers, managers and other departmental personnel in a kind, knowledgeable and friendly manner.10. Functions as a leader for the CNAs. Delegates in a manner that contributes to the excellence of the nursing unit to which LVN is assigned.11. Participates in orientation of new personnel and assists in performance evaluations when needed.12. Consistently follows the policies and procedures of the facility.13. Motivated, independent individual with the ability to organize workload so that all functions are completed appropriately.14. Performs miscellaneous job related duties as assigned. Qualifications: Education• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required• Completion of an accredited Vocational Nursing School program OR become licensed by equivalency requiredExperience• 6 months of experience in a long term care facility preferredLicenses and Certifications• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• LVN - Current State of California Licensed Vocational Nurse License required
