Responsibilities: Responsible for various aspects of care management to ensure that care being rendered is indicated, resources utilized are appropriate, and services provided are in the appropriate setting. Works collaboratively with other departments and physicians to facilitate efficient and appropriate management of all Ambulatory related cases. Qualifications:

• Bachelor's degree in Nursing or related field and two years Care Coordination experience.
• Six years experience in related field may be substituted for experience and education.
• Current CA Registered Nurse License.
• Current BLS certification.
