The Pediatric Sub Acute Clinical Care Coordinator for Pediatric is responsible for participating in the coordination of patient admissions, serving as a liaison between the patient, family members and financial organizations to ensure that services needed are met and are authorized so resident care can be provided. The Care Coordinator is also responsible for ensuring that financial authorizations, PSARR’s, TAR’s and ARF’s are obtained timely and accurately. Also assists in preparation and implementation of a discharge plan for each resident including oversight of process, obtaining needed authorizations, DME's and MD Order. Ensures Infection Control procedures and HIPPA compliance are followed.
Foothill Regional Medical Center is a fully accredited acute care hospital, licensed for 177 beds—120 beds for general acute care, 15 intensive care beds and 42 pediatric sub-acute beds. Our pediatric sub-acute unit, just one of six in California, serves patients from throughout the state. We care for patients up to 21 years of age who need long-term sub-acute care including ventilator care and nutritional support.
- Collaborates with the Patient Access / Central Business Office to ensure that PSARR’s, TAR’s and ARF’s are competed timely, accurately and appropriately to ensure financial reimbursement. Works with the Pediatric Subacute team in assisting with discharge planning including obtaining needed authorizations for continued care, home site visit and family training as needed. Completes the PSARR for all admissions, coordinates the completion of the psychotropic data collection for IDT meetings.
- Collaborates with staff, physicians and members of the healthcare team, ensuring the coordination of care between other disciplines and support staff. Supervises, monitors and delegates patient care activities to appropriate members of the health care team, provides effective leadership and ensures that care is safe and of high quality. Acts as a source of information and articulately responds to questions, providing resources, guidance, and education about treatment plan, plan of care, and medications.
- Empathizes with patient and patient support systems, expresses compassion and understanding for the situation and demonstrates care which is appropriate for the age, and cultural, religious, gender identity, or ethnic background. Engages in caring behaviors, taking conscious ownership for work and takes deliberate/thoughtful actions regarding how to best carry out interactions with the patient, families, physicians, and co-workers.
Required Qualifications:
- Current Licensure as a Registered Nurse in the State of California
- Bachelor of Science in Nursing
- AHA Basic Life Support
- Ability to read, write and communicate effectively in English
- Ability to work variable shifts including evenings, nights, weekends, and holidays
- Ability to maintain flexibility and composure in a constantly changing environment
- Two (2) years of previous clinical experience
- Hospital Fire and Life Safety Card (Los Angeles City Employees only)
Preferred Qualifications:
- AHA Pediatric Advanced Life Support
- Bilingual skills to communicate effectively with patients and families
