Addus Homecare is looking for a Full-Time Director of Centralized Intake for our Home Health Division. The Director of Centralized Intake will be the key process owner for incoming Home Health referrals overseeing multiple locations. This leader will ensure standardization, best practices, and compliance with State guidelines relative to the Home Health benefit. They will also be responsible for the implementation and supervision of the policies, procedures and workflows used by the Intake staff for the direct purpose of obtaining and maintaining eligibility for patients as well as managing all aspects of the patient intake process including overarching clinical standards and practices.

Qualifications:
- 3-5 years of Home Health experience
- Prior Home Health leadership experience preferred
- Current Registered Nursing (RN) licensure required a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC).
- Strong Knowledge of Medicare Home Health regulations, admission criteria and compliance standards
- Demonstrated record of successfully taking on increased responsibility (goal achievement)
- Ambition to grow and advance beyond current position
- Excellent communication and interpersonal skills with demonstrated decision-making ability Must be self-confident, thorough, and prompt in completing assignments and projects
- Passionate, energetic, tenacious and resolute, with a high sense of urgency and a strong drive to produce results
- Strong communication, organizational and interpersonal skills
- Valid driver’s license and proof of insurance is required
What You’ll Do:
- Directs all daily patient referral and intake operations
- Complies with applicable legal requirements, standards, policies, and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA, and Documentation Standards
- Implements ongoing improved work methods and procedures to ensure patients are admitted to accordance with strong clinical practices and policies in a timeliness approach
- Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet customer needs
- Maintain effective fiscal management productivity of team by monitoring metrics and publishing of KPIs Leadership, supervision and support of the Intake Coordinator team
- Provide superior customer service and high-quality Home Health Care advisory
- Supports intake process and team in leading by example and participating in daily work routines and key processes:
- Processing of patient intake and coordination of care, end-to-end
- Key liaison to business development team establishing strong processes of communication
- Actively drives communication processes with patient, family and referral sources / providers
- Complies and is a role model for all areas of the Agency Compliance Program and HIPAA regulations.
- Maintain a high degree of confidentiality at all times due to access to sensitive information
- Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the department
- Follow all Medicare, Medicaid, and HIPAA regulations and requirements
- Abide by all regulations, policies, procedures and standards
- Perform other duties as assigned
